Service Coordination is administered offsite by individual organizations. Service Coordination assists persons with developmental disabilities in accessing supports and services based on their needs and valued outcomes. Service Coordination activities promote person-centered planning, individual choice, and consumer satisfaction. A Service Coordinator works for the individual in need of services and is key to helping the person identify and access desired supports and services.
The Service Coordinator’s activities include advocating and developing an individual service plan and monitoring the implementation of the plan. Service Coordination activities promote self-advocacy, informed decision-making, and self-determination.
